During Office Hours

During office hours, please click on the "Book an Appointment Online Live Chat" picture on the right. Type in your contact information when the form opens up. Then, one of our team members will open a Live Chat session and you can communicate through "texting" or typing. Please call our office if this isn't working for you.


After Office Hours

After office hours, the "Live Chat" won't be available, but you can type a message there, or click on the "Contact Dr. Mann's Team" button below the team photo on the right side bar.


New Patient Registration Forms
